When someone is diagnosed with malignant mesothelioma, the treatment options are limited, but not entirely out of the question, particularly if the individual’s cancer is caught in its early stages. One of the best treatment options is surgery, and this is the option doctors prefer if the individual is in otherwise good health, and the mesothelioma hasn’t metastasized too far.

Palliative Pleurectomy

With pleural mesothelioma, which is found in the lining of the lungs, the preferred type of surgery is typically a pleurectomy. In general, pleurectomy is an operation that aims to remove the pleura, with the goal of alleviating fluid collection in the impacted area. Pleurectomy on its own is primarily to provide relief of the symptoms associated with mesothelioma but isn’t necessarily designed to improve survival rates.

Pleurectomy can be considered more of a palliative care option to get rid of pleural effusions, which are a buildup of fluid. What Is a Pleurectomy/Decortication?

A pleurectomy with decortication describes a procedure in which the impacted lining of the patient’s lung is removed, along with any other tumors that may be present in the chest.

Pleurectomy/decortication is seen as a surgery option to improve survival expectations. People who undergo a P/D have an average life expectancy of anywhere from 9 to 19 months after surgery.

The ideal candidate for a P/D operation is someone with a stage 1 mesothelioma diagnosis who doesn’t have tumors that have spread beyond the lining of the lung, also known as the pleura.

Also worth discussing is the extrapleural pneumonectomy, which was the first surgery option designed to extend survival times for patients with pleural mesothelioma. With this surgery, which is ideal for patients who are stage I or II, an incision is made, and the pleura and pericardium are separated from the wall of the chest and the diaphragm.

The pulmonary vessels that supply the lung affected by mesothelioma are redirected, the affected areas including the lung and pericardium are removed. Then, there is reconstruction using Gore-Tex.

Of these surgeries, the basic pleurectomy is the simplest in most cases, since it’s primarily done to alleviate painful symptoms, while the extrapleural pneumonectomy is the most complex of the surgeries that can be offered to mesothelioma patients.
